The fillable Campaign Contribution Disclosure Report (CCDR) form
is in PDF format. This file will be used for you to copy blank CCDR forms
to a drive of choice on your computer. Prior to making entries on the forms,
it is highly recommended and encouraged you save the CCDR form under a different
file name (candidate's name, office held or seeking, and reporting period;
for example John Wesley Commissioner District 8 3-25-08).
This will greatly assist you in records maintenance and clearly identify to us
who is transmitting the electronic CCDR. For future filings, you may return
to the DeKalb County Voter Registration website and download any additional
blank file CCDR forms needed.
